Position Summary: The MDS Coordinator (Minimum Data Set Coordinator) is responsible for managing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations and facility policies. This role ensures the accurate completion of MDS assessments, proper case mix classification, and appropriate reimbursement through Medicare, Medicaid, and other payers. Working closely with nursing staff, therapy, social services, and administration, the MDS Coordinator drives quality outcomes through precise documentation and thoughtful care planning.
Key Responsibilities: - Oversee and manage the RAI process including MDS assessments, CAAs (Care Area Assessments), and interdisciplinary care planning
- Ensure timely and accurate MDS submissions in compliance with federal (CMS) and state guidelines
- Monitor and audit documentation to support MDS coding and reimbursement accuracy
- Coordinate and lead care plan meetings in collaboration with the interdisciplinary team
- Educate and support nursing and care staff in documentation best practices and regulatory requirements
- Track and analyze resident acuity and case mix data to support financial and clinical planning
- Communicate with billing, therapy, and external reviewers to ensure proper reimbursement and compliance
- Maintain current knowledge of CMS regulations, PDPM, and nursing home survey processes
- Participate in Quality Assurance & Performance Improvement (QAPI) activities
- Maintain confidentiality and comply with HIPAA and facility policies at all times
Required Qualifications: - Active RN license in the state of Wyoming (required)
- Minimum of 2 years of nursing experience, with at least 1 year in long-term care or skilled nursing (MDS experience preferred)
- Strong working knowledge of MDS 3.0, PDPM, RAI guidelines, and care planning processes
- Familiarity with CMS regulations, Medicare/Medicaid billing, and quality measures
- Excellent organizational, communication, and analytical skills
- Proficiency in electronic health records (EHR) and MDS software systems
- Ability to work independently and collaboratively with cross-functional teams
- High attention to detail and commitment to accuracy and compliance
